Referring to the letter of the Anbar Health Department - Department of Public Health, which includes the guidance of the respected Prime Minister on the need for the ministries of Education, Higher Education and Scientific Research to cooperate with the Ministry of Health and Environment to facilitate the taking of anti-virus CORONA vaccines for educational and teaching staff and for the sake of the presidency of the distinguished University of Anbar: represented by Mr. President of Anbar University Professor Dr. Mushtak Talib Saleh Al-Neda respected, please ladies and gentlemen in University of Anbar (exclusively) who wish to vaccinate, to provide us with their data by accurately answering the model below.
